Documentation overhaul with Discount (Markdown implementation).

The standard LuaDoc template is no longer used. Instead, the new
`scripts/markdowndoc.lua` has the template for LuaDoc and `scripts/update_doc`
has the template for the Manual.
Also added README, CHANGELOG, and THANKS files.
